Qualifications

• • A graduate of Bachelor of Science in Nursing 
• • Must be a Registered Nurse 
• • Proficient in Software application, operating system and other relevant skills 
• • With good moral character

2. Duly Accomplished Personal Data Sheet (CS Form No. 212 revised 2025) with work experience sheet. The PDS should duly administered by a competent authority. 

3. Certified true copy of TOR/Diploma 

4. Certified true copy of training certificates 

5. Certified true copy of Certificate of Eligibility/Board Rating/License (if applicable) 

6. Certificate of Previous/current employment with details of Job description (if applicable)
